Membership Agreement

 The member applicant understands and agree:

  1. The membership shall be valid within 1 year from time of purchase.
  2. Only GCASH or Online Bank Transfer shall be valid.
  3. The membership includes:
    1. 1 principal member and 2 dependents. For married individuals,  dependents refer to children below 18 years old or legitimate spouse for married individuals. For single individuals, dependents refer to their parents or siblings below 18 years old. 
    2. 3 Membership cards. 
    3. 3 free medical consultations. Medical consultations may be done online or face to face at Pharmadoo Health Hub located at 2nd Floor West Gateway Complex Brgy. San Rafael Iloilo City by Pharmadoo Physicians.
    4. If face to face consultation is chosen, medical assessment shall be done every first and last Saturdays of the month.
    5. If online consultation is chosen, consultation shall be done during the Physician’s consultation hours.
    6. Medical and Patient History Records annually updated by Pharmadoo Physicians and stored at Pharmadoo Health Hub.
    7. Home health care and home health diagnostics services to be performed by Pharmadoo Partner Physicians and Diagnostics. The services are not free and subject to applicable charges. 
    8. Membership deals posted at the website every month.
  4. The member agrees that all medicines and laboratory requests prescribed by Physicians shall be done by Pharmadoo accredited pharmacies and laboratories.
  5. The member certifies the truthfulness of the data filled up in his membership application form and medical patient history.
  6. His membership application and medical patient history shall be stored at Pharmadoo Headquarters for the duration of his membership.
  7. By purchasing this Membership, the applicant agrees to the collection, use and processing of his/her personal information by Pharmadoo for the purposes of patient record keeping, administration and scheduling of e-consultations and home visits, payment of dues, inclusion of contact information in a member’s directory that will be distributed to members, employees and clients of Bizfix Corporate Hub.
